Your Impact:
As a Medical Social Worker, you'll play a vital role in empowering patients and their caregivers to navigate the challenges they face. Your responsibilities will include:
Comprehensive Assessments: Conducting thorough psychosocial assessments to understand each patient's unique needs, considering their age, medical history, background, and cultural diversity.
Personalized Care Planning: Developing and implementing individualized care plans that address patients' emotional, social, and practical needs.
Referrals and Interventions: Connecting patients with appropriate community resources and providing supportive interventions to improve their health and overall quality of life.
Collaborative Care: Working closely with physicians, nurses, therapists, and other healthcare professionals to ensure seamless and comprehensive care coordination.
Advocacy and Case Management: Acting as a strong advocate for patients, helping them access necessary services and navigate complex healthcare systems.
Building Meaningful Connections: Establishing strong, supportive relationships with patients and their families, providing compassionate care in a one-on-one setting and ensuring their dignity is respected.
Documentation and Reporting: Maintaining accurate and timely documentation of patient assessments, care plans, and interventions.
What We're Looking For:
We're seeking a compassionate and skilled Medical Social Worker who possesses the following qualifications:
A Bachelor's or Master's degree in Social Work from a CSWE-accredited program.
Current and valid Social Work license in the state.
A minimum of two (2) years of experience as a Social Worker, preferably in a community or home health setting.
Proven ability to work effectively with diverse populations, including youth, adults, and geriatric patients.
Strong understanding of community resources and healthcare systems.
Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues.
Critical thinking and problem-solving skills to address complex patient needs.
A positive attitude and a strong team spirit.
Current BLS CPR certification.
Valid driver's license and reliable, insured vehicle for travel to patient homes in various weather conditions.
