Job Description

Position Summary:

This position will be responsible for working with a multidisciplinary team and providing discharge planning services. This includes identifying eligible offenders, developing and tracking a discharge plan for each offender assigned to the caseload. The discharge plan should include all necessary referrals/assessments to ensure a continuum of care upon release. This position is a lead worker and will be assigned cases that require more intense intervention. In addition, this role will be required to provide data for the team as requested.

LMSW – Licensed Master's Social Worker needed for a Discharge Planner role in Osawatomie, KS:

• 13-week direct contract with a high potential to extend.
• Quick feedback and expedited credentialing process with in-house experts.
• 40 hours per week with potential to pick up OT.
• Shift: Mon-Fri. | 8-hour shift times available.

LMSW – Licensed Master's Social Worker Pay:

• Gross weekly pay based on 40 hours: $2,640 - $2,720

Discharge Planner/Social Worker (LMSW) Qualifications:

• Must be a Licensed Master Social Worker ( LMSW ) in the state of Kansas
• One year experience in planning, implementing, and monitoring activities relevant to the agency’s programs. Education may be substituted for experience as determined relevant by the agency.
• Experience with offenders and/or special needs populations.
• Case Management and/or Discharge Planning experience preferred.

Discharge Planner/Social Worker (LMSW) Job Duties:

• Documents patient progress including placement, communication with the mental health centers, guardians, family/significant others.
• Assists patients with completing applications for benefits including CARE assessments, Medicaid, Social Security Applications and Waivers.
• Coordinate placement of patients to the least restrictive environment possible
• Maintain data and information related to processes and impact, and provide the information to the statewide reentry team, as requested by the Reentry Director.
• Other duties as assigned.
