As a NP at Horizon, you will…
- Provide initial psychiatric examinations for individuals admitted to mental health, rehabilitation and recovery programs and provide follow up medication management services.
- Participate in clinical case conference and case reviews to programs to provide consultation and guidance with difficult cases, assures communication in multiple forums with counseling staff to be updated on consumer progress and outstanding issues or concerns.
- Prescribe psychotropic and/or addiction medication to ameliorate symptoms; reviews incoming physical exams to ensure if the information is current and accurate.
- Communicate to primary counselor the need to make referrals for physical exams with consumer's primary care physician.
- Ensure that all confidential and potentially sensitive information is processed, maintained, and utilized in a strictly confidential manner.
- Review and sign all treatment plans for patients enrolled in program--- as needed.
- Document all services provided in the case record. Must be literate with electronic health records.

What makes you a great candidate? We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:
- Master's Degree in a Nurse Practitioner Program and National Board Certification in specialty area required.
- Current license to practice as a Nurse Practitioner in New York State, required.
- Current license to practice as a Registered Professional Nurse in New York State, required.
- 3 years’ experience required, specifically with individuals with mental health needs.
Location :
This position would be located at 3345 Southwestern Blvd, Orchard Park NY. This position may require you to work out of 2 Horizon locations.
Hours: This position is full-time from Monday - Friday.
Compensation: Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $65.00 - $80.00 per hour. Starting pay may be impacted based on experience and education including DNP. This position includes a bonus earning opportunity, with an annual max potential of $2,400, based on volume of service visits provided and cash equivalent opt out benefits. Compensation adjustments are evaluated annually.
Physical demands associated with this position include: - Stationary posture with frequent standing/sitting alternation.
- Mobility between nearby worksites and within facilities.
- Mobility required involving repetitive wrist, hand, and finger movements.
- Lifting objects up to 20 pounds.
